To make an appointment in the near future, please give us a call at (516) 343-7146 and we can set up your appointment right away. You may also submit this form to schedule your appointment, but please allow some time for us to confirm the appointment. Credit card information is required to hold your appointment. Cancellation policy: If you need to cancel or reschedule your appointment, please give us a call at least 24 hours before your appointment. Not appearing to your appointment without a cancellation call will incur a no show fee at an amount equivalent to the service you booked, up to $100. For every 30 minute delay, a late fee of $25 will apply, or your appointment may be cancelled.
